Is there a vertical version of your image I can use to play vertical shmups on my vertical cab?

I know there's a way change settings in RetroArch to rotate the games sideways, but what about the main menu(s)??

No separate version and definitely do not go messing with individual emulator settings. Just navigate to the service menu and select the vertical option you want and the system should reconfigure itself.

You should only need to change the menu to a vertical theme, everything else should be automated.
